Penegra is, used for the treatment of pulmonary arterial hypertension and erectile dysfunction i.e. lack of ability to maintain or achieve an erection to perform sexual activity in men because of inadequate flow of blood in the penis. It can make an erection through relaxation of the penile the blood vessels, thereby escalating blood flow into the penis, which is essential to maintain an erection in response to sexual stimuli. It should be noted, that Penegra cannot work without sexual stimulation and it does not increase libido. It primarily works through inhibition of cGMP-specific phosphodiesterase type 5 (PDE5). This is an enzyme that carries forward cGMP degradation, which in turn regulates blood flow in the penis. The manufacturer of Penegra is Zydus Alidac.

Qualitative and quantitative composition: Each film coated tablet of Penegra contains 25, 50 or 100 mg sildenafil citrate equivalent to the same amount of sildenafil. The coat further contains 0.834, 1.667 and 3.334 mg of lactose (as monohydrate) respectively.

Pharmacological action: Penegra works through inhibition of cGMP-specific phosphodiesterase type 5 (PDE5). This is an enzyme that carries forward cGMP degradation, which in turn regulates blood flow in the penis. But it cannot work without sexual stimulation and cannot be used to increase libido.

Therapeutic indications: Penegra is indicated for the treatment of erectile dysfunction in adult men who cannot maintain sufficient penile erection for satisfactory sexual performance. For Penegra to work sexual stimulation is necessary. It cannot be used in adult women and children and it does not increase the libido.

Contraindications: Penegra cannot be given to patients having hypersensitivity to it. Further, it has hypotensive effects of nitrates thus cannot be co-administered with nitric oxide donors. It should not be administered in patient with severe cardiac disorder and others who had been advised against sexual activity.

Penegra is also contraindicated in patients suffering from loss of vision in one eye due to non-arteritic anterior ischaemic optic neuropathy (NAION). Effect of Penegra is still not fully of stroke, and hereditary retinal disorder.

Special warnings and precautions for use: Administration of Penegra should be done with caution if the patient has cardiovascular risk factors. Severe cardiovascular problems have been noted after use of Penegra (cerebrovascular haemorrhage, myocardial infarction, sudden cardiac death, ventricular arrhythmia, transient ischaemic attack, hypertension and hypotension, unstable angina).

Patients suffering from priapism and anatomical deformation (cavernosal fibrosis, angulation or Peyronie's disease) of the penis proper treatment should be done instead of using Penegra as it may cause permanent loss of sexual potency. Prolonged erections are another aspect that need to be monitored and if erection persists for more than four hours immediate assistance of doctors should be taken.

It has hypotensive effects of nitrates thus cannot be coadministered with nitric oxide donors. Concomitant use with other PDE5 inhibitors or other treatments for erectile dysfunction and, with ritonavir, alpha blockers (can have systematic hypotension) is not advisable. Penegra is also contraindicated in patients suffering from loss of vision in one eye due to non-arteritic anterior ischaemic optic neuropathy (NAION). Penegra can also cause peptic ulcer and bleeding disorder, though no specific case has been reported. The coating contains lactose, so cannot be used for patients suffering from galactose intolerance. Penegra is not for adult women and children.

Interaction with other medicinal products: Inhibitors of cytochrome P450 (CYP) isoforms 3A4 (major route) and 2C9 (minor route) through which metabolism of Penegra takes place, can cause reduction in the rate of Penegra clearance. Similarly inducers of these isoenzymes will increase the clearance of Penegra.

Nicorandil (hybrid of potassium channel activator and nitrate) can cause severe interaction with Penegra because of the nitrate component.

The drugs, including antacids (aluminium or magnesium hydroxide); Dipyridamole/ theophylline (non-specific phosphodiesterase inhibitors); tolbutamide; warfarin; acetyl salicylic acid; HIV protease inhibitors (ritonavir/ saquinavir) have no direct interaction with Penegra and do not hamper the bioavailability of Penegra. Penegra is also not affected by consumption of alcohol as per studies made (up to 80 mg/dl alcohol levels in blood).

Undesirable effects: Patients treated with Penegra have several adverse reactions that may include blurred vision, dizziness, nasal congestion, hot flush, headache, cyanopsia, nausea, and dyspepsia.

Interesting facts:

  • One should only consider receiving Penegra tablet when having a plan to perform sex; it should not be planned as a regular medicine.
  • Though Penegra is very effective in getting erection in the presence of sexual stimuli, it cannot increase the libido.
