Introduction: Furosemide (Lasix 40mg) is a quick-acting loop diuretic with a pronounced diuretic effect. It is used for edema syndrome of various origins, including in chronic heart failure, liver cirrhosis, nephrotic syndrome, as well as for relief of hypertensive crisis and forced diuresis.

Pharmacology: Furosemide is a rapid-acting diuretic, a derivative of sulfonamide. Mechanism of action of Furosemide is associated with the blockade of reabsorption of sodium and chloride ions in the ascending limb of Henle's loop. Lasix also promotes excretion of potassium, calcium and magnesium.
Furosemide has strong diuretic, natriuretic, chloruretic properties. The diuretic effect is already observed during the first hour of the intake, the maximum is achieved within 1-2 h after administration, the duration of diuretic effect is 6-8 hours.
Indications:
- edema syndrome of different genesis (chronic heart failure, syndrome of portal hypertension, edematous ascitic syndrome, nephrotic syndrome)
- renal failure, nephrotic syndrome, severe hypernatremia, hypercalcemia, and hypermagnesemia
- acute left ventricular failure (cardiac asthma, pulmonary edema)
- cerebral edema
- hypertension
- forced diuresis
Contraindications:
- acute glomerulonephritis
- end-stage of renal failure, oliguria lasting for more than 24 h, anuria
- hepatic coma
- diabetic coma
- violations of water-salt metabolism and acid-base balance: hypokalemia, hyponatremia, hypovolemia, alkalosis
- gout, hyperuricemia
- decompensated mitral or aortic stenosis
- hypertrophic cardiomyopathy with obstruction of the outflow tract of left ventricle
- hypotension
- myocardial infarction
- hypersensitivity to furosemide and other sulfonamides
- pregnancy, breastfeeding
Adverse effect: The Lasix 40mg may cause the following side-effects:
- in long-term use or high doses use: hypovolemia, dehydration, hypokalemia, hyponatremia, chloropenia, hypomagnesemia, alkalosis
- cardiovascular system: hypotension, including postural, collapse, cardiac arrhythmia, thromboembolism, thrombophlebitis
- digestive tract: dry mouth, nausea, vomiting, diarrhea
- transient hyperuricemia
- dermatitis
- dizziness, muscle weakness, seizures, confusion
Interactions: Furosemide should not be used with the following groups of drugs or preparations:
- hypoglycemic medications
- pressor amines
- cardiac glycosides
- glucocorticoids
- aminoglycosides
- cephalosporins
- ethacrynic acid
- Li+ drugs
- NSAIDs
- uricosuric medicines
- antifungal agents
- ACE inhibitors
- curare-like medicaments
