Ciprofloxacin or Generic Cipro is a second-generation fluoroquinolone which acts as a broad spectrum antibiotic against bacterial infections. Its spectrum of action includes both Gram negative and Gram positive bacteria. Infections of respiratory tract, gastrointestinal tract and urinary trac can be cured with Ciprofloxacin can be used to cure abdominal, urinal and respiratory tract infections. Apart from its broad spectrum of antibacterial activity, Ciprofloxacin is highly acclaimed admirable tissue penetration and availability as both oral and intravenous formulation.

Generic Cipro

Qualitative and quantitative composition: Ciprofloxacin is available as film-coated tablets of strengths 250, 500 and 750mg. Oral suspension of Ciprofloxacin (Generic Cipro) is available in strengths of 5% and 10%.

Pharmacological action: Ciprofloxacin acts against bacteria by inhibiting the enzymatic action of topoisomerase II and IV or DNA gyrase which play key role in bacterial DNA replication, recombination, DNA repair and in transcription.

Therapeutic indications: In case of adult patients, Ciprofloxacin is indicated for lower respiratory tract infections (pneumonia caused by various bacteria); kidney & urinary tract infections; gastrointestinal tract infections (peritonitis) and infections of skin, bones, joints & soft tissue including sepsis. It can also be administered post exposure of Bacillus anthracis (causative agent of anthrax). In case of children Generic Cipro is given for Cystic fibrosis, pyelonephritis and severe urinary tract infections. Ciprofloxacin cannot be used in any viral infection and resistant bacterial infections, including Peptostreptococcus, Nocardia asteroids, Ureaplasma urealyticum and Enterococcus faecium.

Contraindications: Administration of Ciprofloxacin should be stopped if cases of hypersensitivity in receiving patients are noted against ciprofloxacin or any other quinolone chemotherapeutic. Concurrent use of tizanidine and ciprofloxacin must be avoided as serum level concentration of tizanidine can be increased, leading to drowsiness and hypotension.

Special warnings and precautions for use: Generic Cipro may cause tendinitis and hypoglycaemia to its recipients thereby indicating the requirement of proper monitoring. In case of severe infections administration of ciprofloxacin should be combined with other suitable antibacterial agents.

Ciprofloxacin is not recommended for treatment of Streptococcus pneumoniae infections, genital tract infections (N. gonorrhoeae), patients with cardiac disorders (due to QT prolongation), children and adolescents (unless no other options are available/severe infections as they can produce arthropathy immature bone joints), patients hypersensitive to Ciprofloxacin, patients suffering from diarrhoea after receiving Ciprofloxacin, patients with hepatic disorder, tendinitis, patients having CNS-disorders and in patients suffering from photosensitisation (patients receiving Ciprofloxacin should avoid excessive sunlight).

Ciprofloxacin inhibits Cytochrome P450 enzymes at a moderate level, thus products (theophylline, methylxantines, caffeine, duloxetine, clozapine) of this enzymatic pathway should be administered to the patients receiving Ciprofloxacin after careful monitoring.

Interaction with other medicinal products: Ciprofloxacin may cause false negative results of  Mycobacterium spp. culture test by suppressing the mycobacterial growth. Ciprofloxacin administration should be made with extreme caution in case the patients have the following situations: have class IA or III antiarrhythmics; receiving mineral supplements due to chelation complex formation (calcium, iron, magnesium, etc.); having dairy products and fortified drinks alone (milk, fortified juice, yoghurt); receiving Omeprazole, Theophylline, Other xanthine derivatives, NSAID, Cyclosporin, Vitamin K antagonists, Glibenclamide, Methotrexate, Metoclopramide, Tizanidine, Duloxetine, Lidocaine, Clozapine and Sildenafil; during pregnancy and lactation. Patients receiving Ciprofloxacin may also feel it difficult to drive and operate heavy machinery due to its effect on CNS and hence should be duly cautioned. Patients should never take alcohol while receiving Generic Cipro.

Undesirable effects: The most frequent and repeatating adverse effects associated with administration of Ciprofloxacin are abdominal pain with diarrhoea, vomiting, nausea and taste distortion. These effects are not very severe and quite common with any macrolide antibacterial drugs. During clinical trials, patients with or without a previous history of mycobacterial infections experienced similar side effects.

Interesting facts:

  • In case of Ciprofloxacin the first discovered member of the class works best. This is a rarity where subsequent members of the class failed to provide better results and compete with the mother one on the basis of lower price only.
  • Among fluoroquinolone, Generic Cipro became the first drug to be approved for children of the age group 1-17 years.
